Task not processed, "explorer failed to queue task"

The error "explorer failed to queue task" indicates that a task was created and sent to the Explorer, but it didn't respond within 5 minutes.

This usually happens when an Explorer is offline for a period of time, and a task is created and assigned during that time period. It can also be caused by transient network issues, particularly if the WebSocket connection from the explorer to the hub is interrupted but not closed.
